Premium Health is providing the COVID-19 and Flu vaccines at two designated Primary Care locations.
Showing your insurance card or providing insurance information is not mandatory to get a vaccine. Clients will not incur out-of-pocket payments.
Vaccines are available by appointment during general office hours. We also welcome walk-ins during select vaccine walk-in hours. Please see below for details.
